Medical tests ruled out cancer and sexually transmitted diseases, revealing a candida infection that could indicate a weakened immune system. While testing negative for illegal drugs, he tested positive for a mix of benzodiazepines and opiates, medications he used for anxiety and sleep.

Carter's health was a concern due to malnourishment and being underweight at 115 pounds (52 kg) despite a height of 6 feet (1.8 m). He was advised to undergo drug rehabilitation and medical supervision. He entered Alo House in Malibu for treatment and reported weight improvement to 160 pounds (73 kg) by February 2018.

In 2019, Carter and his mother appeared on the reality series Marriage Bootcamp: Family Edition, aiming to mend strained relationships through unconventional therapy. In another appearance on The Doctors that year, Carter disclosed his diagnoses of schizophrenia and bipolar disorder.

On November 5, 2022, Aaron Carter passed away at 34 years old in his residence in Lancaster, California. His body was discovered in the bathtub by a housekeeper. However, the Los Angeles County Medical Examiner-Coroner later confirmed that Carter's demise was accidental, resulting from drowning due to inhaling difluoroethane and taking alprazolam.
